The Annapurna Circuit Trek with Tilicho Lake is a 14-day trekking route in Nepal that combines the classic Annapurna Circuit with a side trip to Tilicho Lake (4,919 m). The trail circles the Annapurna Massif, passing through river valleys, traditional villages, alpine terrain, and the Mustang area before crossing Thorong La Pass (5,416 m).

Adding Tilicho Lake changes the character of the trek. Instead of staying only on the main circuit trail, you take a quieter mountain route through rocky slopes and high alpine terrain toward the glacier-fed lake at 4,919 metres. Surrounded by mountains and away from the busier trail, Tilicho Lake offers one of the most peaceful sections of the Annapurna Circuit. It requires extra effort, but it takes you into a part of the route many trekkers never reach.

The trek begins with a drive from Kathmandu to the lower Annapurna area before starting the walk from Dharapani. The first days follow the Marsyangdi River through terraced farmland, waterfalls, forests, and Gurung and Magar villages. As the trail climbs through Chame and Pisang, the green hills gradually give way to open valleys, rocky slopes, and the high mountain environment around Manang.

Manang is where altitude starts to become a serious part of the trek. Many trekkers feel strong during the lower sections but notice the change once they move above 3,500 metres. I always tell trekkers that this is the point where patience matters more than speed. Spending enough time here helps your body adjust before heading toward Tilicho Lake and Thorong La Pass.

From Manang, the route turns toward Khangsar, Tilicho Base Camp, and Tilicho Lake. This section is quieter than the main circuit and includes rougher trails, rocky paths, and exposed sections. The walk requires more attention, especially on narrow paths and open mountain slopes where weather can change quickly. It takes you into an area that many Annapurna Circuit trekkers never reach.

After returning from Tilicho Lake, the trail continues toward Thorong La Pass at 5,416 metres, the highest point of the trek. The climb usually begins early in the morning when the weather is more stable. Crossing the pass takes you from Manang into Mustang, where the scenery changes again with dry valleys, cliffs, and villages influenced by Tibetan culture.

The descent from Thorong La leads to Muktinath, a place visited by both Hindu and Buddhist pilgrims for generations. From here, the route continues through Mustang, passing stone villages, wide valleys, and the Kali Gandaki area before moving toward the lower Annapurna hills.

The trek also gives you a chance to experience the communities that have lived along this route for generations. You pass through Gurung, Manangi, and Thakali villages, see Buddhist monasteries, prayer wheels, and chortens, and stay in family-run teahouses where meals, rest, and evenings are spent around the same routine shared by trekkers and local families.

The Annapurna Circuit Trek with Tilicho Lake requires an Annapurna Conservation Area Permit (ACAP) and TIMS card, which I arrange before the trek begins. No climbing permit is required because this route does not involve technical climbing.

Most trekking days involve around 5 to 7 hours of walking, with longer days during the Tilicho Lake visit and Thorong La Pass crossing. The challenge comes from the combination of distance, altitude, and changing trail conditions. A steady pace and proper acclimatisation matter much more than trying to walk fast.

This is the exact 14-day itinerary I follow on every Annapurna Circuit and Tilicho Lake trek I run. Starting in Kathmandu, we drive to Besisahar and on to Chame, then trek through Pisang and Manang before going to Tilicho Lake, one of the highest lakes in the world. From there, we rejoin the main circuit through Yak Kharka and Thorong High Camp to cross Thorong La Pass to Muktinath, before driving back to Pokhara and on to Kathmandu.
